How Did You Hear About Me?

In the below, I ask you to name the referral source - a couple of reasons why. For the most part, it's polite to say thank you.

In no way are there any kick-backs for referrals. In other words, if someone recommends me - I don't pay them a fee and there is no mutual arrangement to back each other. 

This means someone referring me is doing so because they like what I do and/or know I do good work (or they could be related - I have a big family)
